The Derealize mission is the final quest in the Taken Path exotic quest. It's a lengthy one on its own, so let's get started with how to easily beat the mission.
Derealize mission explained in Destiny 2
The first section of the mission is super simple. You only need to clear waves of enemies, three in total.
There's a small gimmick with a massive Taken eye that will inflict a debuff called Suffocating Terror. If you let it reach upwards of 17 you'll start losing health, but it's nothing to worry about as this isn't a restricted area.
You need to defeat two tormentors and a Follower of Power miniboss to complete this area. Towards the end, a concealed Taken portal will be revealed.
Once you go through the portal, you'll reach the Trenchway, where the second phase of the exotic mission will start. Here you'll have to defeat Vrhiisk, Seeker of Power
Do damage to him until the main mechanic of the encounter starts. The boss will become invincible during this period. First, you need to defeat six Blight Sentinels. These are like regular Taken cabal.
Then, Taken ogres will appear. You need to eliminate one for it to drop Taken essence for you to commute with, similar to how it was done in the Curse quests.

Time to become one with the Taken
Now that you're empowered, defeat Taken until you've powered up the Curse. Once done, you'll need to dunk the essence in a Taken blight which will be highlighted on your screen.
You'll get a notice that says you are "Torn Between Dimensions," but nothing that will actually impact gameplay. Repeat the process to make the boss' shield disappear.

You'll need to do it a total of four times. Eventually, the boss will leave the area and spawn you in a different location, this is where you'll deal the finishing blow and the next phase of the mission will start.
Derealize exotic mission puzzle solution
This puzzle seems much more complicated than it actually is. There are four symbols and a small plate you need to stand on. In front of you, there will be a rune that will display a symbol.

Pay close attencion to the simbols
You need to select the symbols in the order displayed from left to right and move in the direction the plate is pointing towards. Don't rotate within the plate, use your directional keys or whatever it is you may be using without turning around.
So for instance, the first plate points to the right. If the symbol displayed on the rune matches the next symbol you need to select, move right. If it doesn't, let the time run out and stand on the next place. Be mindful of where you need to move this time in case the symbols match.
After you complete this puzzle, there will be a small traversal section. Follow the marker to collect a Taken Osseus Fragment. Drop down through the hole that was cleared after the Taken blight disappeared and you'll drop down to another traversal section.
Climb the Taken infested walls and head out of the area. You'll see a giant Shrieker-like ship. Go to it and enter the other side of the wall. Follow the path while dodging the small Taken blips that explode on contact. Towards the end, you'll find the first of three crystals you need to destroy.

You'll need to shoot three crystals
Head out and you'll be on the other side of the massive open-ended area. This time, the giant Shrieker will be covered in Taken blights. Just a little hint to let you know you're on the right track.
Make your way to the other side of the wall once more. You can use the mid-sized Taken blights as makeshift stairs.
When you reach the other side, look up and to the right to find the second crystal. The third and final one is below the platforms you're standing.

The crystals are relatively easy to spot
Find the right angle to shoot it and the gate above should open. From here, follow the marker and you'll eventually be able to get the Barrow-Dyad exotic SMG.
The final boss fight is relatively simple. It's got a reality warp gimmick but nothing to worry about. There are technically two bosses, one for each reality, but that's about it for difficulty.
Eventually, you'll seal the beast and get yourself the Barrow-Dyad for good this time and the ability to find its Catalyst via subsequent missions.
